The William E. Flippin Sr. Leadership Award Application

The following requires participation verification on the organization’s letterhead indicating your leadership position, (i.e. president, secretary, treasurer, etc.) dates of your service, and sponsor(s) signature. (Applications with missing sponsor signatures on verification letters will be considered incomplete applications.)
